Lifeisshort,andArtlong;thecrisisfleeting;experienceperilous,anddecisiondifficult。Thephysicianmustnotonlybepreparedtodowhatisrighthimself,butalsotomakethepatient,theattendants,andexternalscooperate。

2。Indisordersofthebowelsandvomitings,occurringspontaneously,ifthematterspurgedbesuchasoughttobepurged,theydogood,andarewellborne;butifnot,thecontrary。Andsoartificialevacuations,iftheyconsistofsuchmattersasshouldbeevacuated,dogood,andarewellborne;butifnot,thecontrary。One,then,oughttolooktothecountry,theseason,theage,andthediseasesinwhichtheyareproperornot。

3。Intheathletae,embonpoint,ifcarriedtoitsutmostlimit,isdangerous,fortheycannotremaininthesamestatenorbestationary;

andsince,then,theycanneitherremainstationarynorimprove,itonlyremainsforthemtogetworse;forthesereasonstheembonpointshouldbereducedwithoutdelay,thatthebodymayagainhaveacommencementofreparation。Neithershouldtheevacuations,intheircase,becarriedtoanextreme,forthisalsoisdangerous,butonlytosuchapointastheperson’sconstitutioncanendure。Inlikemanner,medicinalevacuations,ifcarriedtoanextreme,aredangerous;andagain,arestorativecourse,ifintheextreme,isdangerous。

4。Aslenderrestricteddietisalwaysdangerousinchronicdiseases,andalsoinacutediseases,whereitisnotrequisite。Andagain,adietbroughttotheextremepointofattenuationisdangerous;andrepletion,whenintheextreme,isalsodangerous。

5。Inarestricteddiet,patientswhotransgressaretherebymorehurt(thaninanyother?);foreverysuchtransgression,whateveritmaybe,isfollowedbygreaterconsequencesthaninadietsomewhatmoregenerous。Onthisaccount,averyslender,regulated,andrestricteddietisdangeroustopersonsinhealth,becausetheybeartransgressionsofitmoredifficultly。Forthisreason,aslenderandrestricteddietisgenerallymoredangerousthanonealittlemoreliberal。

6。Forextremediseases,extrememethodsofcure,astorestriction,aremostsuitable。

7。Whenthediseaseisveryacute,itisattendedwithextremelyseveresymptomsinitsfirststage;andthereforeanextremelyattenuatingdietmustbeused。Whenthisisnotthecase,butitisallowabletogiveamoregenerousdiet,wemaydepartasfarfromtheseverityofregimenasthedisease,byitsmildness,isremovedfromtheextreme。

8。Whenthediseaseisatitsheight,itwillthenbenecessarytousethemostslenderdiet。

9。Wemustformaparticularjudgmentofthepatient,whetherhewillsupportthedietuntiltheacmeofthedisease,andwhetherhewillsinkpreviouslyandnotsupportthediet,orthediseasewillgivewaypreviously,andbecomelessacute。

10。Inthosecases,then,whichattaintheiracmespeedily,arestricteddietshouldbeenjoinedatfirst;butinthosecaseswhichreachtheiracmelater,wemustretrenchatthatperiodoralittlebeforeit;butpreviouslywemustallowamoregenerousdiettosupportthepatient。

11。Wemustretrenchduringparoxysms,fortoexhibitfoodwouldbeinjurious。Andinalldiseaseshavingperiodicalparoxysms,wemustrestrictduringtheparoxysms。

12。Theexacerbationsandremissionswillbeindicatedbythediseases,theseasonsoftheyear,thereciprocationoftheperiods,whethertheyoccureveryday,everyalternateday,orafteralongerperiod,andbythesuperveningsymptoms;as,forexample,inpleuriticcases,expectoration,ifitoccuratthecommencement,shortenstheattack,butifitappearlater,itprolongsthesame;andinthesamemannertheurine,andalvinedischarges,andsweats,accordingastheyappearalongwithfavorableorunfavorablesymptoms,indicatediseasesofashortorlongduration。

13。Oldpersonsendurefastingmosteasily;next,adults;youngpersonsnotnearlysowell;andmostespeciallyinfants,andofthemsuchasareofaparticularlylivelyspirit。

14。Growingbodieshavethemostinnateheat;theythereforerequirethemostfood,forotherwisetheirbodiesarewasted。Inoldpersonstheheatisfeeble,andthereforetheyrequirelittlefuel,asitwere,totheflame,foritwouldbeextinguishedbymuch。Onthisaccount,also,feversinoldpersonsarenotequallyacute,becausetheirbodiesarecold。

15。Inwinterandspringthebowelsarenaturallythehottest,andthesleepmostprolonged;attheseseasons,then,themostsustenanceistobeadministered;forasthebellyhasthenmostinnateheat,itstandsinneedofmostfood。Thewell—knownfactswithregardtoyoungpersonsandtheathletaeprovethis。

16。Ahumidregimenisbefittinginallfebrilediseases,andparticularlyinchildren,andothersaccustomedtoliveonsuchadiet。

17。Wemustconsider,also,inwhichcasesfoodistobegivenonceortwiceaday,andingreaterorsmallerquantities,andatintervals。Somethingmustbeconcededtohabit,toseason,tocountry,andtoage。

18。Invalidsbearfoodworstduringsummerandautumn,mosteasilyinwinter,andnextinspring。

19。Neithergivenorenjoinanythingtopersonsduringperiodicalparoxysms,butabstractfromtheaccustomedallowancebeforethecrisis。

20。Whenthingsareatthecrisis,orwhentheyhavejustpassedit,neithermovethebowels,normakeanyinnovationinthetreatment,eitherasregardspurgativesoranyothersuchstimulants,butletthingsalone。

21。Thosethingswhichrequiretobeevacuatedshouldbeevacuated,wherevertheymosttend,bytheproperoutlets。

22。Wemustpurgeandmovesuchhumorsasareconcocted,notsuchasareunconcocted,unlesstheyarestrugglingtogetout,whichismostlynotthecase。

23。Theevacuationsaretobejudgedofnotbytheirquantity,butwhethertheybesuchastheyshouldbe,andhowtheyareborne。Andwhenpropertocarrytheevacuationtodeliquiumanimi,thisalsoshouldbedone,providedthepatientcansupportit。

8。Whenapersonafteradiseasetakesfood,butdoesnotimproveinstrength,itindicatesthatthebodyusesmorefoodthanisproper;

butifthishappenwhenhedoesnottakefood,itistobeunderstoodevacuationisrequired。

9。Whenonewishestopurge,heshouldputthebodyintoafluentstate。

10。Bodiesnotproperlycleansed,themoreyounourishthemoreyouinjure。

11。Itiseasiertofillupwithdrinkthanwithfood。

12。Whatremainsindiseasesafterthecrisisisapttoproducerelapses。

13。Personsinwhomacrisistakesplacepassthenightprecedingtheparoxysmuncomfortably,butthesucceedingnightgenerallymorecomfortably。

14。Influxesofthebowels,achangeofthedejectionsdoesgood,unlessthechangebeofabadcharacter。

15。Whenthethroatisdiseased,ortubercles(phymata)formonthebody,attentionmustpaidtothesecretions;foriftheybebilious,thediseaseaffectsthegeneralsystem;butiftheyresemblethoseofahealthyperson,itissafetogivenourishingfood。

22。Diseaseswhicharisefromrepletionarecuredbydepletion;

andthosethatarisefromdepletionarecuredbyrepletion;andingeneral,diseasesarecuredbytheircontraries。

23。Acutediseasecometoacrisisinfourteendays。

24。Thefourthdayisindicativeoftheseventh;theeighthisthecommencementofthesecondweek;andhence,theeleventhbeingthefourthofthesecondweek,isalsoindicative;andagain,theseventeenthisindicative,asbeingthefourthfromthefourteenth,andtheseventhfromtheeleventh。

25。Thesummerquartansare,forthemostpart,ofshortduration;

buttheautumnalareprotracted,especiallythoseoccurringneartheapproachofwinter。

27。Weshouldnottrustameliorationsindiseaseswhentheyarenotregular,norbemuchafraidofbadsymptomswhichoccurinanirregularform;forsucharecommonlyinconstant,anddonotusuallycontinue,norhaveanyduration。

28。Infeverswhicharenotaltogetherslight,itisabadsymptomforthebodytoremainwithoutanydiminutionofbulk,ortobewastedbeyondmeasure;fortheonestateindicatesaprotracteddisease,andtheotherweaknessofbody。

29。Ifitappearthatevacuationsarerequired,theyshouldbemadeatthecommencementofdiseases;attheacmeitisbettertobequiet。

30。Towardthecommencementandendofdiseasesallthesymptomsareweaker,andtowardtheacmetheyarestronger。

31。Whenapersonwhoisrecoveringfromadiseasehasagoodappetite,buthisbodydoesnotimproveincondition,itisabadsymptom。

32。Forthemostpart,allpersonsinillhealth,whohaveagoodappetiteatthecommencement,butdonotimprove,haveabadappetiteagaintowardtheend;whereas,thosewhohaveaverybadappetiteatthecommencement,andafterwardacquireagoodappetite,getbetteroff。

33。Ineverydiseaseitisagoodsignwhenthepatient’sintellectissound,andheisdisposedtotakewhateverfoodisofferedtohim;butthecontraryisbad。

34。Indiseases,thereislessdangerwhenthediseaseisonetowhichthepatient’sconstitution,habit,age,andtheseasonareallied,thanwhenitisonetowhichtheyarenotallied。

35。Inalldiseasesitisbetterthattheumbilicalandhypogastricregionspreservetheirfullness;anditisabadsignwhentheyareveryslenderandemaciated;inthelattercaseitisdangeroustoadministerpurgatives。

36。Personsingoodhealthquicklylosetheirstrengthbytakingpurgativemedicines,orusingbadfood。

37。Purgativemedicinesagreeillwithpersonsingoodhealth。

38。Anarticleoffoodordrinkwhichisslightlyworse,butmorepalatable,istobepreferredtosuchasarebetterbutlesspalatable。

39。Oldhavefewercomplaintsthanyoung;butthosechronicdiseaseswhichdobefallthemgenerallyneverleavethem。

40。Catarrhsandcoryzainveryoldpeoplearenotconcocted。

41。Personswhohavehadfrequentandsevereattacksofswooning,withoutanymanifestcause,diesuddenly。

50。Thosethingswhichonehasbeenaccustomedtoforalongtime,althoughworsethanthingswhichoneisnotaccustomedto,usuallygivelessdisturbance;butachangemustsometimesbemadetothingsoneisnotaccustomedto。

51。Toevacuate,fillup,heat,cool,orotherwise,movethebodyinanywaymuchandsuddenly,isdangerous;andwhateverisexcessiveisinimicaltonature;butwhateverisdonebylittleandlittleissafe,moreespeciallywhenatransitionismadefromonethingtoanother。

52。Whendoingeverythingaccordingtoindications,althoughthingsmaynotturnoutagreeablytoindication,weshouldnotchangetoanotherwhiletheoriginalappearancesremain。

53。Thosepersonswhohavewaterydischargesfromthebowelswhentheyareyoung,comeoffbetterthanthosewhohavedry;butinoldagetheycomeoffworse,forthebowelsinagedpersonsareusuallydriedup。

1。Thechangesoftheseasonmostlyengenderdiseases,andintheseasonsgreatchangeseitherofheatorofcold,andtherestagreeablytothesamerule。

2。Ofnatures(temperaments?),somearewell—orill—adaptedforsummer,andsomeforwinter。

3。Ofdiseasesandages,certainofthemarewell—orill—adaptedtodifferentseasons,places,andkindsofdiet。

4。Intheseasons,whenduringthesamedaythereisatonetimeheatandatanothertimecold,thediseasesofautumnmaybeexpected。

5。Southwindsinducedullnessofhearing,dimnessofvisions,heavinessofthehead,torpor,andlanguor;whentheseprevail,suchsymptomsoccurindiseases。Butifthenorthwindprevail,coughs,affectionsofthethroat,hardnessofthebowels,dysuriaattendedwithrigors,andpainsofthesidesandbreastoccur。Whenthiswindprevails,allsuchsymptomsmaybeexpectedindiseases。

6。Whensummerislikespring,muchsweatingmaybeexpectedinfevers。

7。Acutediseasesoccurindroughts;andifthesummerbeparticularlysuch,accordingtotheconstitutionwhichithasgiventotheyear,forthemostpartsuchdiseasesmaybeexpected。

8。Inseasonswhichareregular,andfurnishtheproductionsoftheseasonattheseasonabletime,thediseasesareregular,andcomereadilytoacrisis;butininconstantseasons,thediseasesareirregular,andcometoacrisiswithdifficulty。

9。Inautumn,diseasesaremostacute,andmostmortal,onthewhole。Thespringismosthealthy,andleastmortal。

10。Autumnisabadseasonforpersonsinconsumption。

11。Withregardtotheseasons,ifthewinterbeofadryandnortherlycharacter,andthespringrainyandsoutherly,insummertherewillnecessarilybeacutefevers,ophthalmies,anddysenteries,especiallyinwomen,andinmenofahumidtemperament。

12。Ifthebutthespringdryandnortherly,womenwhosetermofdeliveryshouldbeinspring,haveabortionsfromanyslightcause;

andthosewhoreachtheirfulltime,bringforthchildrenwhoarefeeble,anddiseased,sothattheyeitherdiepresently,or,iftheylive,arepunyandunhealthy。Otherpeoplearesubjecttodysenteriesandophthalmies,andoldmentocatarrhs,whichquicklycutthemoff。

13。Ifthesummerbedryandnortherlyandtheautumnrainyandsoutherly,headachesoccurinwinter,withcoughs,hoarsenesses,coryzae,andinsomecasesconsumptions。

14。Butiftheautumnbenortherlyanddry,itagreeswellwithpersonsofahumidtemperament,andwithwomen;butotherswillbesubjecttodryophthalmies,acutefevers,coryzae,andinsomecasesmelancholy。

15。Oftheconstitutionsoftheyear,thedry,uponthewhole,aremorehealthythantherainy,andattendedwithlessmortality。

16。Thediseaseswhichoccurmostfrequentlyinrainyseasonsare,protractedfevers,fluxesofthebowels,mortifications,epilepsies,apoplexies,andquinsies;andindry,consumptivediseases,ophthalmies,arthriticdiseases,stranguries,anddysenteries。

17。Withregardtothestatesoftheweatherwhichcontinuebutforaday,thatwhichisnortherly,bracesthebody,givingittone,agility,andcolor,improvesthesenseofhearing,driesupthebowels,pinchestheeyes,andaggravatesanypreviouspainwhichmayhavebeenseatedinthechest。Butthesoutherlyrelaxesthebody,andrendersithumid,bringsondullnessofhearing,heavinessofthehead,andvertigo,impairsthemovementsoftheeyesandthewholebody,andrendersthealvinedischargeswatery。

18。Withregardtotheseasons,inspringandinthecommencementofsummer,childrenandthosenexttotheminagearemostcomfortable,andenjoybesthealth;insummerandduringacertainportionofautumn,oldpeople;duringtheremainderoftheautumnandinwinter,thoseoftheintermediateages。

19。Alldiseasesoccuratallseasonsoftheyear,butcertainofthemaremoreapttooccurandbeexacerbatedatcertainseasons。

20。Thediseasesofspringare,maniacal,melancholic,andepilepticdisorders,bloodyflux,quinsy,coryza,hoarseness,cough,leprosy,lichenalphos,exanthematamostlyendinginulcerations,tubercles,andarthriticdiseases。

21。Ofsummer,certainofthese,andcontinued,ardent,andtertianfevers,mostespeciallyvomiting,diarrhoea,ophthalmy,painsoftheears,ulcerationsofthemouth,mortificationsoftheprivyparts,andthesudamina。

24。Inthedifferentagesthefollowingcomplaintsoccur:tolittleandnew—bornchildren,aphthae,vomiting,coughs,sleeplessness,frightsinflammationofthenavel,waterydischargesfromtheears。

25。Attheapproachofdentition,pruritusofthegums,fevers,convulsions,diarrhoea,especiallywhencuttingthecanineteeth,andinthosewhoareparticularlyfat,andhaveconstipatedbowels。

26。Topersonssomewhatolder,affectionsofthetonsils,incurvationofthespineatthevertebranexttheocciput,asthma,calculus,roundworms,ascarides,acrochordon,satyriasmus,struma,andothertubercles(phymata),butespeciallytheaforesaid。

27。Topersonsofamoreadvancedage,andnowonthevergeofmanhood,themostofthesediseases,and,moreover,morechronicfevers,andepistaxis。

28。Youngpeopleforthemostparthaveacrisisintheircomplaints,someinfortydays,someinsevenmonths,someinsevenyears,someattheapproachtopuberty;andsuchcomplaintsofchildrenasremain,anddonotpassawayaboutpuberty,orinfemalesaboutthecommencementofmenstruation,usuallybecomechronic。

29。Topersonspastboyhood,haemoptysis,phthisis,acutefevers,epilepsy,andotherdiseases,butespeciallytheaforementioned。

30。Topersonsbeyondthatage,asthma,pleurisy,pneumonia,lethargy,phrenitis,ardentfevers,chronicdiarrhoea,cholera,dysentery,lientery,hemorrhoids。

31。Tooldpeopledyspnoea,catarrhsaccompaniedwithcoughs,dysuria,painsofthejoints,nephritis,vertigo,apoplexy,cachexia,pruritusofthewholebody,insomnolency,defluxionsofthebowels,oftheeyes,andofthenose,dimnessofsight,cataract(glaucoma),anddullnessofhearing。

1。Wemustpurgepregnantwomen,ifmattersbeturgid(inastateoforgasm?),fromthefourthtotheseventhmonth,butlessfreelyinthelatter;inthefirstandlaststagesofpregnancyitshouldbeavoided。

2。Inpurgingweshouldbringawaysuchmattersfromthebodyasitwouldbeadvantageoushadtheycomeawayspontaneously;butthoseofanoppositecharactershouldbestopped。

3。Ifthematterswhicharepurgedbesuchasshouldbepurged,itisbeneficialandwellborne;butifthecontrary,withdifficulty。

4。Weshouldratherpurgeupwardinsummer,anddownwardinwinter。

5。Aboutthetimeofthedog—days,andbeforeit,theadministrationofpurgativesisunsuitable。

6。Leanpersonswhoareeasilymadetovomitshouldbepurgedupward,avoidingthewinterseason。

7。Personswhoaredifficulttovomit,andaremoderatelyfat,shouldbepurgeddownward,avoidingthesummerseason。

8。Wemustbeguardedinpurgingphthisicalpersonsupward。

9。Andfromthesamemodeofreasoning,applyingtheoppositeruletomelancholicpersons,wemustpurgethemfreelydownward。

10。Inveryacutediseases,ifmattersbeinastateoforgasm,wemaypurgeonthefirstday,foritisabadthingtoprocrastinateinsuchcases。

11。Thosecasesinwhichtherearetormina,painsabouttheumbilicus,andpainsabouttheloins,notremovedeitherbypurgativemedicinesorotherwise,usuallyterminateindrydropsy。

12。Itisabadthingtopurgeupwardinwinterpersonswhosebowelsareinastateoflientery。

13。Personswhoarenoteasilypurgedupwardbythehellebores,shouldhavetheirbodiesmoistenedbyplentyoffoodandrestbeforetakingthedraught。

14。Whenonetakesadraughtofhellebore,oneshouldbemadetomovemoreabout,andindulgelessinsleepandrepose。Sailingontheseashowsthatmotiondisordersthebody。

15。Whenyouwishthehelleboretoactmore,movethebody,andwhentostop,letthepatientgetsleepandrest。

16。Helleboreisdangeroustopersonswhosefleshissound,foritinducesconvulsion。

17。Anorexia,heartburn,vertigo,andabittertasteofthemouth,inapersonfreefromfever,indicatethewantofpurgingupward。

18。Painsseatedabovethediaphragmindicatepurgingupward,andthosebelowit,downward。

19。Personswhohavenothirstwhileundertheactionofapurgativemedicine,donotceasefrombeingpurgeduntiltheybecomethirsty。

20。Ifpersonsfreefromfeverbeseizedwithtormina,heavinessoftheknees,andpainsoftheloins,thisindicatesthatpurgingdownwardisrequired。

21。Alvinedejectionswhichareblack,likeblood,takingplacespontaneously,eitherwithorwithoutfever,areverybad;andthemorenumerousandunfavorablethecolors,somuchtheworse;whenwithmedicineitisbetter,andavarietyofcolorsinthiscaseisnotbad。

22。Whenblackbileisevacuatedinthebeginningofanydiseasewhatever,eitherupwardordownward,itisamortalsymptom。

23。Inpersonsattenuatedfromanydisease,whetheracuteorchronic,orfromwounds,oranyothercause,iftherebeadischargeeitherofblackbile,orresemblingblackblood,theydieonthefollowingday。

24。Dysentery,ifitcommencewithblackbile,ismortal。

25。Blooddischargedupward,whateverbeitscharacter,isabadsymptom,butdownwarditis(more?)favorable,andsoalsoblackdejections。

26。Ifinapersonillofdysentery,substancesresemblingfleshbedischargedfromthebowels,itisamortalsymptom。

27。Inwhatevercasesoffeverthereisacopioushemorrhagefromwhateverchannel,thebowelsareinaloosestateduringconvalescence。

28。Inallcaseswhatever,biliousdischargesceaseifdeafnesssupervenes,andinallcasesdeafnessceaseswhenbiliousdischargessupervene。
